We would like to thank you for taking part in the new process of accepting athletic participation paperwork online. This automated process will allow Dallastown Area School District to be more efficient as well as saving valuable resources.

• To access the online forms, click the Start Online Forms button below.

• Click on the form name and fill out the information requested.

• Click HERE for instructions on how to create a Parent Account.

• If you have previously created an account, you may sign in and complete the forms. If you have not previously created an account, you may create one using your personal email address.

• You must have your student ID number available as it is required on the form.

• To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box.

• Once you have filled out all the information you will have the opportunity to print the document.

• If you are completing the forms using an account, you can check the status of each form submitted by logging into your account.

To access a physical and health history copy, click on the Download/Print tab to print off a blank copy. Take this with you to your child’s physical appointment and after completion please return upload it to your account in Rank One.
